Negative Pressure(VAC) Dressings:
We regularly perform a thorough evaluation of the wound to determine its size, depth, and characteristics. Our nurses are well trained to carefully apply the Negative Pressure(VAC) dressing and ensure it adheres securely to the skin, creating an airtight seal. We consistently assess the wound and dressing for any signs of complications or issues, such as leaks or skin irritation.
Care of the Ventilated Patient at Home:
Our nurses are adequately trained in ventilator operation, maintenance, and emergency procedures. They will properly set up the ventilator and continuously monitor its function, alarms, and patient-ventilator synchrony. Respiratory status, including vital signs, lung sounds, and oxygen saturation are regularly monitored and effective airway is maintained through routine suctioning, following strict aseptic techniques.
Comprehensive Nursing Assessment:
Delivering holistic care, our community nursing assessments prioritize individualized healthcare plans. We meticulously assess physical, emotional, and social factors, promoting wellness. Our comprehensive approach ensures clients receive the highest quality care tailored to their unique needs, fostering independence and well-being.
Complex Wound Care:
We specialize in complex wound care. Our expert team conducts thorough wound assessments, using evidence-based practices. We prioritize optimal healing, reducing pain, and enhancing quality of life. We aim to offer care that promotes recovery and restores the individual's well-being.
Catheter Care:
Our approach in catheter care management emphasizes infection prevention, proper catheter selection, routine assessments, securement, aseptic technique, closed drainage systems, daily hygiene, preventing urinary stasis, patient education, and timely removal when no longer needed. Adherence to these principles minimizes complications, ensures patient safety, and enhances overall care quality.
Bowel Management:
Our Bowel management involves thorough assessments, promoting a balanced diet and fluid intake, adjusting medications, educating patients, ensuring easy toilet access, regular monitoring, and individualized care. It emphasizes conservative measures before medication and aims to enhance patient comfort and quality of life through personalized strategies, and in communication with other healthcare providers.
PEG & NGT Management:
We aim to maintain a sterile environment during tube handling, and practice rigorous hand hygiene. We also follow prescribed feeding schedules and formulas as directed by healthcare providers, and administer medications through the tube as necessary, crushing or dissolving them if required. We routinely assess the patient for complications, intolerance, or infection, and provide patients and caregivers with training on tube care, signs of complications, and emergency procedures.
Tracheostomy Care:
Our caregivers and healthcare providers are trained in tracheostomy care, including emergency procedures. We emphasize on regularly inspecting and cleaning the tracheostomy tube to prevent blockages, and replace it as recommended by healthcare providers. We use strict aseptic techniques when handling the tracheostomy tube and when providing suction, dressing changes, or tracheostomy care. We ensure to monitor and maintain proper humidification and suctioning to keep airways clear and maintain adequate oxygen levels.
